A population-based chronic disease management program designed to improve health outcomes. This initiative is a partnership among West Virginia University, the Office of Health Services Research, and the NTTC. We have integrated the Chronic Disease Electronic Management System (CDEMS) into HEALTHeWV’s electronic health records (EHR) system, giving clinics the opportunity to participate in the Health Disparities Collaborative.
